Youths must be enrolled as members of OCW before registering for programs. You’ll only need to fill this out once, but please let us know if anything changes. No personal or contact information will be shared with others. A red asterisk indicates that an answer is required. Questions about race and ethnicity are optional. We ask only because some funders may inquire about the demographics of the population we serve.
Once you’ve enrolled a student, you can register him or her for programs.
